Raffle Prize Form

Prize submission for the UPCOMING CONNECTIONS EVENT

Members are invited to provide a raffle prize. Please provide your contact info and the details for the prize you will provide to the raffle. Please keep in mind attendees will be from all over the US and beyond; shipping should be included in your prize if it is a physical item. We will put you in touch with the winner of your item so that you can coordinate delivery. One prize submission per person. Multiple prize submissions will be combined into one prize.
